At Ascend Transition Services Foundation, we offer comprehensive nursing services to support individuals with intellectual and developmental disabilities who require specialized care.

These services are designed to promote independence, ensure safety, and improve quality of life for individuals and their families. Our team works closely with each individual’s healthcare providers to ensure that the right level of care is delivered in a comfortable and familiar environment.

Our nursing services

Private Duty Nursing

Provides continuous, individualized nursing care in the home or community, addressing complex medical needs that require a higher level of attention. This service is available for individuals who need active nursing interventions for extended periods.

Skilled Nursing

Offers part-time or intermittent nursing care for individuals with medical needs that cannot be managed by personal care alone. Provided by registered or licensed practical nurses, this service focuses on tasks like medication management, wound care, and health monitoring.

Skilled Respite

Gives caregivers temporary relief by providing skilled care for individuals under 21 with developmental disabilities. Trained nurses offer medical supervision and support, allowing caregivers to take a break while ensuring the individual’s medical needs are met.

APD-approved provider

MAP (Medication Administration Procedure) ADP Training

Ascend Transition Services Foundation is an approved provider of the Medication Administration Procedure (MAP) Training, a requirement for direct care staff working with individuals served by Florida’s Agency for Persons with Disabilities (APD).

Our certified trainers deliver high-quality instruction that ensures staff are fully prepared to administer medications safely and in compliance with APD standards.

MAP Training Includes:

Overview of APD Medication Standards

Training on APD's rules and procedures for medication administration, documentation, and safety.

Hands-On Practice

Participants receive guided practice in administering oral, topical, and inhaled medications using simulated scenarios.

Three-Step Process Mastery

Focused instruction on the "three rights" and "three checks" process to ensure accuracy and prevent errors.

Recordkeeping & Documentation

Training on proper use of MARs (Medication Administration Records) and required documentation procedures.

Competency Testing

Participants must demonstrate proficiency through written and practical evaluations to receive certification.

Who Should Attend

Direct Support Professionals (DSPs), group home staff, personal care assistants, and other caregivers working in APD-funded settings.

Certification

Upon successful completion, participants receive MAP certification valid for 1 year, meeting all APD compliance requirements.
