Who does Ascend Transition Services serve?
We empower individuals with intellectual and developmental disabilities (IDD/DD) in Southwest Florida to achieve independence and community inclusion, supporting individuals at all levels of need.
What programs and services do you offer?
We offer Life Skill Programs (four levels, from Companion and Supported Employment to Adult Day Training and Prevocational Services), Home Services (Personal Care Assistance and Respite Care), and Nursing Care (Private Duty Nursing, Skilled Nursing, Skilled Respite, and MAP medication administration training).
Where is Ascend Transition Services located?
We are located at 506 SW 47th Terrace, Cape Coral, FL 33914.
What are your hours?
We are open Monday through Friday, 7:30 AM to 3:30 PM, and closed on weekends.
What ages do your programs serve?
Our life skills and personal care services are designed for adults (generally 21 and older), and respite care is available for individuals under the age of 21.
How can I support Ascend?
You can become a Community Builder by committing to one of three annual donation plans, or make a one-time gift. Every dollar donated goes toward keeping our programs, training, and care efforts alive.
Do you provide MAP (medication administration) training?
Yes. Ascend Transition Services Foundation is an approved provider of MAP (Medication Administration Procedure) training for direct care staff working with individuals served by Florida’s Agency for Persons with Disabilities (APD).
