Ascend Transition Services is dedicated to providing families with post- secondary transition support through various Life-Skill Adult Day Training Programs, nursing services, companion support, pre-vocational programs, employment support, and community based instruction.

We strive to be a comprehensive, compelling, and dedicated service. While remaining compassionate to the various needs of our members.

We are a diverse group of professionals ready to support our community.

Our vision is to create an inclusive community in SWFL by teaching families, businesses, corporations, and organizations strategies that support adults with intellectual or developmental disabilities.

Our mission is to provide post-secondary transition services to adults with intellectual or developmental disabilities by creating long-lasting opportunities within the SWFL community.

Our founders are dedicated advocates who believe in the power of inclusion and the strength that comes from embracing every individual's abilities. Both are dedicated to empowering the SWFL community. Bridget’s unwavering commitment to individuals with intellectual/developmental disabilities motivated her to exit the classroom setting and create a space where individuals can safely transition into the community and adulthood. Rick's background as a First Responder/Fire Fighter motivated him to co-found Ascend and create new cultures within medical and emergency settings. Both are dedicated to providing life long services!
